People who identify with moderate to severe mental illness and complex needs engage in these services. We will help you identify your unique recovery goals and create an agreed recovery plan. Bridges approach Bridges approach focuses on how your condition impacts your life. We have been delivering community-based adult mental health services for over 20 years in regional Queensland. Our focus is on supporting people who identify as having a mental illness. Such condition may be, schizophrenia, bi-polar disorder, depression, anxiety, and/or personality disorders. We also support those with mental ill-health without a diagnosis who experience barriers, issues, or have adverse life events. It is our goal to support individuals to achieve their personal goals, a healthy mind, and live a positive, and proactive life. Our service Our commitment is to work alongside you in whatever way you require, providing varying levels of support as needed. Our community-based mental health team comprises of professionally trained and qualified mental health professionals. They support a broad base of clients, including those with: Schizophrenia Personality disorders Depression Anxiety Post-traumatic stress disorder (PTSD) Suicidal ideation And other mental health conditions We also offer case management for people with long-term mental health diagnosis and complex needs. To accommodate, there are three programs that our clients will be referred into: Individual Recovery Support Program (IRSP) National Psychosocial Support Program (NPSP) Family Mental Health Support Service (FMHSS) A client engaged with these services can expect a service period of about one year (12 months). However, this is dependent on the individual and their needs. Nonetheless, our clients can count on their Bridges professional to walk alongside them every step of the way. The Bridges ‘No Wrong Door’ policy means we’ll help you find the right solution. Referrals & fees Bridges adult mental health team receives referrals into the respective program from various agencies. These include: National Psychosocial Support Program (NPSP) receives referrals from: Your hospital, health service, or GP You, as a person who chooses to self-refer Individual Recovery Support Program (IRSP) only receives referrals from your local Hospital and Health Service (HHS).
