Staying on Track

The Staying on Track program supports young people in the youth justice system to reconnect with family, education, and community. In the Wide Bay region, it is delivered by Bridges Health & Community Care, who work with referred young people to provide guidance, practical support, and pathways that help them get back on track.

About Staying on Track

The Staying on Track program is funded by the Queensland Department of Youth Justice and Victim Support and delivered locally by Bridges Health & Community Care across Hervey Bay, Bundaberg, Gympie and the North Burnett. The program is designed to support young people aged 10–17 as they transition from detention back into their families and communities.

Staying on Track provides up to 12 months of tailored mentoring and practical support, with the first six months focused on intensive supervision. Bridges works with each young person, their family, and Youth Justice to develop a personalised Service Response Plan (SRP). These plans may include pathways to education and training, employment preparation, cultural mentoring, driving lessons, volunteering, health and disability supports, and pro-social activities. Regular outreach, home visits, and after-hours support help young people stay connected, motivated, and safe during this critical period of reintegration.

 

Our role extends beyond individual mentoring. We work alongside families to strengthen relationships, provide practical help such as access to housing and Centrelink support, and connect young people with community services. Every plan is grounded in cultural safety, family engagement, and collaboration with Youth Justice and other agencies. By building strong, trusting relationships and responding quickly to each young person’s needs, the program aims to reduce reoffending and help young people build positive futures.
